Dream Street provides camping programs for children and young adults with chronic and life threatening illnesses.

Our camp is very special and you will have the opportunity to change someone’s life and possibly your own.
In this online process, you will complete a volunteer application, background screening forms and a medical history.

Once accepted, we will be sending a link for a background check via email to complete the application process.
Understand that in conjunction with my application for employment, DREAM STREET FOUNDATION will use the services of an outside agency to research and verify the information I have provided on my application for employment including my personal background, character, professional standing, work history and qualifications. This agency will provide a report to DREAM STREET FOUNDATION. DREAM STREET FOUNDATION uses Backgrounds Online, a consumer-reporting agency, as an agent to perform background verifications.
Backgrounds Online will utilize various sources of information it deems appropriate including but not limited to: Workers Compensation records, Department of Motor Vehicle records, criminal conviction records, current and former employers, military records, education records, professional and personal references. I request, authorize and consent to the release and disclosure of any and all information including but not limited to the above to DREAM STREET FOUNDATION and Backgrounds Online.
I request, authorize and consent to the procurement of an Investigative Consumer Report and understand that it may contain information about my background, mode of living, character, personal characteristics and general reputation. This authorization in original or copy form shall be valid for two years from the date indicated next to my signature. According to the Fair Credit Reporting Act, I will be notified by DREAM STREET FOUNDATION if employment is denied because of information obtained from a Consumer Reporting Agency. Additionally, I understand that if requested within 60 days, I will be given a full and accurate disclosure as to the nature and substance of all information provided to DREAM STREET FOUNDATION. I further understand that when requesting a copy of the report, proper identification will be required and I should direct my request to: Backgrounds Online, 1915 21st Street, Sacramento, CA 95811, phone: 800-838-4804.
LAW ENFORCEMENT AGENCIES AND OTHER ENTITIES FOR POSITIVE IDENTIFICATION PURPOSES REQUIRE THE FOLLOWING INFORMATION WHEN CHECKING PUBLIC RECORDS. IT IS CONFIDENTIAL AND WILL NOT BE USED FOR ANY OTHER PURPOSES. I HEREBY RELEASE DREAM STREET FOUNDATION AND ITS AGENTS, BACKGROUNDS ONLINE AND ALL PERSONS, AGENCIES, AND ENTITIES PROVIDING INFORMATION OR REPORTS ABOUT ME FROM ANY AND ALL LIABILITY ARISING OUT OF THE REQUEST FOR OR RELEASE OF ANY OF THE ABOVE MENTIONED INFORMATION OR REPORTS.
By typing your name on the signature line of this agreement, you are indicating your intent to conduct transactions with Camp Dream Street by electronic means and that your typed name above constitutes your electronic binding signature to this document within the meaning of California Civil Code Section 1633.2(h).
Do Not Bring any sleep medications to camp.
Important:Please notify the Dream Street office if you have been exposed to any communicable disease during the three weeks prior to camp.
For good and valuable consideration from Dream Street Foundation and Camp Dream Street, the adequacy and receipt of which I hereby acknowledge, I hereby expressly grant to said Dream Street Foundation and Camp Dream Street, or any third party either of them may authorize, and to their employees, agents and assigns, the right to photograph me (or my child/ward) and/or make recordings of my/his/her voice, and the right to use pictures, recordings and other reproductions of my/his/her physical likeness or voice (as the same may appear in any still-camera photographs, videotape, and/or motion picture film) for any advertising, promotion and/or fundraising, without any further compensation, All such photographs, videotapes, motion picture films, and recordings, and all negatives or masters thereof, shall be the sole and exclusive property of Dream Street Foundation and Camp Dream Street.
Pursuant to California Civil Code Section 25.8, I hereby authorize the Director or Nurse of Camp Dream Street to consent to any X-ray, examination, anesthetic, medical or surgical diagnosis or treatment, or hospital care to be rendered to me or my said child/ward under the general or special provisions of the Medical Practice Act, or to consent to any X-ray examination, anesthetic, dental or surgical diagnosis or treatment, or hospital care to be rendered to me or my said child/ward by a dentist licensed under the provisions of the Dental Practice Act. This authorization shall be effective whether such diagnosis, treatment or care is rendered at the office of said physician or dentist, at a hospital, at Camp Dream Street, or elsewhere, and shall remain effective while I am or my said child/ward is enroute to or from or involved or participating in any program or activity at Camp Dream Street, unless earlier revoked by me in writing and delivered to the Director.
On my own behalf and on behalf of my child/ward, I hereby expressly release, discharge and hold harmless Camp Dream Street and the Dream Street Foundation, and their respective agents, employees, officers, and representatives, of and from any liability or responsibility arising from any damage, loss, or injury sustained by me or my child/ward while traveling to or from Camp Dream Street, while attending Camp Dream Street or any trips or other activities sponsored by the Dream Street Foundation, or while staying in any accommodations provided or arranged by Camp Dream Street or the Dream Street Foundation, other than any such liability or responsibility which may arise as a result of their gross negligence or willful misconduct. This release includes within its scope any damage, loss or injury sustained as a result of any ordinary negligence, whether active or passive on the part of Camp Dream Street, the Dream Street Foundation, or any of their agents, employees or representatives.
The foregoing release is to be construed in accordance with the laws of the State of California. It is intended to release claims, which are as yet unknown. Accordingly, I hereby wave, on my own behalf and on behalf of my child/ward, the provisions of the California Civil Code Section 1542 which provides:
“A general release does not extend to claims which the creditor does not know or suspect to exist in his favor at the time of executing the release, which if known by him must have materially affected his settlement with the debtor.”
